Obtaining Your Medical Records
A. Request For Medical Records To Be Mailed To Your Home Address On Record:
- Complete the Release of Protected Health Information Form.
- Mail to:

- Please Note - It will Take 5-7 days to process the request.

NOTE: Virginia Law permits a charge for your records. Healthport has been contracted to provide this service and will invoice you directly. Virginia Rates are $0.25 PER PAGE PLUS POSTAGE AND HANDLING. PRE-PAYMENT MAY BE REQUIRED PRIOR TO RELEASE OF RECORDS.
B. Request For Records To Be Sent To A Physician
- Complete the Release of Protected Health Information Form.
- Fax To: 1-855-226-6070
OR
- Complete and sign a records release form from your physician’s office. A copy of the request for medical records can be faxed to your physician office. Requests from physician's offices for patient medical records will be expedited as needed. There will be no charge for records being faxed to a physician.
